1. Getting Started
You will need to complete each hospital’s application for clinical privileges. We can help you obtain these applications.

You will need to assemble the following copies of the following up-to-date documents to include with your application: In addition, you will need to supply the hospitals’ credentialling offices with three reference letters which attest to your clinical competence. If you are in a residency training program, your program director must write one of your reference letters.

Prior to starting work at many of the facilities, you will need to become a BCBS, Medicare and Medicaid provider. Our office can assist you in preparing those applications.

We request that you supply our office with copies of all of your credentialling information. We like to keep it on hand so we can facilitate the credentialling process. Don’t delay or procrastinate even if some items are incomplete. Send what you have now, and forward the rest as it becomes available.

The following agencies can assist you in obtaining the required documentation, including licensure, certification and insurance coverage.
2. Massachusetts Medical License
To obtain a Massachussets Medical License application visit the Commonwealth of MA Board of Registration in Medicine web site at http://www.massmedboard.org/.

Some tips for applying:
  • It may take two weeks to get the application to your mailing address.
  • Complete the application in full. Do not leave anything pertaining to you blank. Be careful to give all the information with the correct spelling and zip codes, and please make sure you provide all required documentation, as not doing this will delay the process.
  • American educated doctors need to make sure all specified information is attached.
  • Foreign Medical Graduates or doctors educated outside the USA will need to provide additional information.
  • Fee is a [whopping] $600. This is to be sent with the application

Keep A Complete Copy of the Application with Attachments and a Copy of the Check or Money Order. You will need to submit a copy of this to each hospital. We cannot believe the number of doctors who disregard the advice to keep a copy of their application. The Board charges $5 for a copy of your application and this causes a needless delay.
3. Controlled Substance Certificate
Applications for a Massachusetts Controlled Substance Certificate can be downloaded in PDF format from the Mass Department of Public Health website at http://www.state.ma.us/dph/dcp/dcpmcsr.htm. Also see the Drug Control Program website at http://www.state.ma.us/dph/dcp/ for more information.

Some tips for applying:
  • Fill out the application completely. Return the application to the instructed address with a copy of your Massachusetts Medical License. Any incorrect or missing information will delay the processing of your application and the receipt of your certificate.
  • Fee of $150.00, as a check or money order, needs to be sent in with the application.
  • It may take 4 weeks to process the application and send your certificate to your mailing address.

Keep a Complete Copy of the Application with Attachments and a Copy of the Check or Money Order.
4. Federal DEA Certificate
You can register online for a Federal DEA Certificate at their web site at http://www.deadiversion.usdoj.gov/. Follow instructions at the site.

Some tips for applying:
  • To apply you must be:
    • Fully licensed as a medical doctor;
    • Have a MA DPH Controlled Substance Certificate
  • Fill the application out completely; do not leave anything blank. Send all requested attachments.
  • Fee of $540.00, payable on line.
  • Upon receipt of the application in Washington:
    • Your information is entered into their database
    • Washington office sends it back to Boston: where the information is verified, and the application is approved.
    • A request is then sent to Washington for a Certificate to be generated.
    • Washington sends the DEA certificate directly to the doctor’s mailing address.
  • The certificate is issued in 3 year cycles (approximately)
  • The DEA will send you a notice 45 days before your expiration date as long as they have the correct address. Please be aware if you change your address or practice location You Must Notify the DEA with the change immediately. There is no charge for a change of address.
  • The cost for a new or renewal certificate is $540.00.
5. Malpractice Insurance
To obtain Malpractice Insurance call:

One option is PROMUTUAL. There are other options, including the American Psychiatric Association.

Some tips for applying:
  • Call and request an application; fill it out and return to the instructed address.
  • Promutal will evaluate the application, develop a quote and sent out a quote letter.
  • If the quote is agreeable, the doctor signs and returns with a check or money order, which is either the first installment or the full amount.
  • Once the check is received, the policy becomes effective.
  • A doctor can either do business directly with the insurance company or have an agent handle their business.
